AI SOAP note generator for therapists
Paste or dictate what happened in the session. Get an editable SOAP, BIRP, DAP or GIRP note in under two minutes — with client identifiers stripped in your browser before a single word leaves the page.
Your note appears here
Four editable sections, ready to paste into SimplePractice, TherapyNotes or any EHR — or export straight to Word.
Automate clinical progress notes in seconds, not hours
The average clinician spends six to nine hours a week on documentation, most of it rewriting the same session into a structure the record expects. That is time you are not spending on clients, and it is the first thing that slips when the day runs long.
Back-to-back sessions, no backlog
Between two clients you have three minutes. Dictate the recap, generate, move on. Notes stop spilling into the evening.
One structure, every session
Your whole caseload comes out in the same format, which is exactly what auditors and supervisors look for.
Nothing you have to clean up
Section content is editable inline, so you are signing something you wrote, not something you are rewriting.
100% client-side anonymisation — zero PHI on external servers
Most AI note tools ask you to trust their database. We removed the database from the equation. Before a request is made, a redaction pass replaces names you listed, phone numbers, email addresses, dates, street addresses, record numbers and SSNs with placeholders — and shows you the result so you can confirm it.
What leaves your browser is the redacted text and nothing else. We cannot leak what we never received.
Read the full HIPAA statementYou write the truth, unfiltered
Use real first names in the box if you want — the next step removes them.
The browser strips identifiers
A visible preview lists every replacement before you generate. You can add names the pattern rules would miss.
Only placeholders travel
Provider-side, a note reads "client" or "[Client A]" — never an identity.
Nothing is written back
No notes table, no transcripts, no training on your text. Metering rows record tokens and format only.
How it works: from voice dictation to EHR-ready documentation
1. Speak or paste your notes
Dictate a two-minute recap or paste the bullet points you already typed. No template fields, no dropdowns.
2. Identifiers are stripped locally
A redaction pass runs in your browser and shows you exactly what it removed before anything is transmitted.
3. The model structures it
Your note is reorganised into the clinical format you picked, using only what you wrote — nothing is invented.
4. Edit, copy or export
Fix a line inline, copy plain text straight into your EHR, or download a .docx with clean section breaks.
Supported formats: SOAP, BIRP, DAP and GIRP notes
Same input, four structures. Switch the format tab and regenerate — you do not rewrite your notes to match a template.
SOAP
Subjective · Objective · Assessment · Plan
The default progress note format used by most outpatient practices and accepted by every major EHR.
BIRP
Behavior · Intervention · Response · Plan
Behavioral-health standard that documents what the clinician did and how the client responded.
DAP
Data · Assessment · Plan
A shorter three-section note for practices that fold subjective and objective data together.
GIRP
Goal · Intervention · Response · Plan
Goal-anchored variant used in settings that require explicit linkage to the treatment plan.

Questions therapists ask before signing up
Is this HIPAA compliant?+
We are deliberately not a covered entity and we do not sign business associate agreements — because no protected health information reaches us. Redaction runs inside your browser, and only the redacted text is sent to the model provider. That makes the risk you are transferring very small, but it also means the tool cannot be your compliance officer: check your own policy on external AI tools before you use it.
Do you store my notes?+
No. There is no notes table in our database. We record metering facts only — which format, which model, how many tokens, how long it took — so we can bill accurately. Your raw text and the generated note exist in your browser session and nowhere else.
What happens to the note after I close the tab?+
It is gone. If you want a copy, export the .docx or paste it into your EHR before you close the tab. There is no recovery, and that is intentional.
How accurate is the assessment section?+
The model is instructed to restate only what you wrote and to mark anything missing as not documented. It never assigns diagnoses, doses, or test scores. Every note is editable, and the tool flags information a complete note would normally contain but your input did not include.
Which formats are supported?+
SOAP, BIRP, DAP and GIRP. Pick the one your practice or supervisor expects; the same input produces the matching structure without you rewriting anything.
Can I use dictation?+
Yes, in Chrome and Edge. Dictation uses your browser speech engine, which may send audio to the browser vendor, so it is off by default and the tool warns you the first time you enable it. Redaction only covers text — say "client" or an initial rather than a full name when speaking.
How many notes do I get for free?+
Ten notes every month, renewed automatically, no card required. Paid plans start at $9 for 100 notes per month.
