What goes in a DAP note
DAP merges Subjective and Objective into a single Data section. For a straightforward session where what the client said and what you observed point the same direction, the separation adds words without adding information.
It is the format most clinicians default to when the caseload is full and the note has to be finished before the next client walks in.
