What goes in a BIRP note
BIRP puts the intervention at the centre. Where SOAP describes the session, BIRP documents the clinical action you took — the technique, the modality, the prompt — and then what the client did in response to it.
That makes BIRP the stronger choice in settings where someone will later ask "what treatment did this client receive?" rather than "what did this client report?"
Behavior
Presenting behavior and reported concerns
Intervention
Techniques and modalities applied
Response
Client response to the intervention
Plan
Follow-up and next session focus
