What goes in a SOAP note
SOAP is the default structure for outpatient documentation: what the client reported, what you observed, what you concluded, and what happens next. It maps cleanly onto almost every psychotherapy progress note, which is why insurers, supervisors and EHR templates all expect it.
The work is not the thinking — it is the conversion. You already know what happened in the session; you spend the time reformatting it into four headings that satisfy a reviewer who was not there.
Subjective
Client-reported experience, symptoms and history
Objective
Observable presentation, affect, behavior, interventions used
Assessment
Clinical impression, progress toward goals, risk
Plan
Next steps, homework, scheduling, referrals
Worked examples
Read a full note before you write your own — the structure is easier to copy from an example than from a template.
