What goes in a GIRP note
GIRP opens with the treatment-plan goal the session addressed, which makes medical necessity visible at a glance. If your documentation is reviewed against a written treatment plan — by an insurer, a court, or an internal audit — that first line does most of the work.
It is also the format that makes drift obvious: if you cannot name the goal a session served, that is clinically useful information.
Goal
Treatment-plan goal addressed this session
Intervention
Techniques and modalities applied
Response
Client response and observed change
Plan
Follow-up and next session focus
